Dr. Nidah Khan, MD
Dr. Nidah Khan, MD, is a distinguished board-certified internal medicine physician, celebrated for her expertise in primary care, lifestyle medicine, weight management, and aesthetics. Inspired by her mother, a revered pediatrician in Pakistan, Dr. Khan pursued her residency at the esteemed Berkshire Medical Center in Pittsfield, MA. Fueled by an unwavering commitment to patient wellness, she champions the integration of lifestyle management, focusing on nutrition and fitness to combat chronic diseases effectively.
Nestled in the scenic landscapes of Western Massachusetts with her husband and three children, Dr. Khan finds solace in leisurely walks and the simple pleasure of losing herself in a captivating book by a cozy winter fire.

Talal Khan, MD
Talal Khan, MD, is a board-certified internist specializing in obesity medicine, preventive care, and chronic disease management at Personal Primary Care in Springfield, Massachusetts.
He believes lifestyle changes and healthy choices are crucial to helping his patients manage their chronic illnesses, lose weight, and avoid future complications. It is this holistic and functional approach to medicine that drives his practice.
Dr. Khan has always been passionate about primary care. He practiced in rural Maine, treating patients from all walks of life — Harvard professors and lobster fishermen alike — for five years before relocating to Western Massachusetts, his home for the last decade.
He received his medical degree from Rawalpindi Medical College in Rawalpindi, Pakistan, and completed his residency at SUNY in Brooklyn, New York. Dr. Khan has medical staff privileges at both Baystate Medical Center and Mercy Medical Center in Springfield.
When he’s not helping his patients reach their wellness goals, Dr. Khan enjoys spending time outdoors with his wife, also a physician, and their three children.
Avery Pedro, PA-C
Avery Pedro, PA-C, is a board certified physician assistant who is passionate about lifestyle medicine, preventative medicine, and longitudinal care with goals to maximize health and wellness. She has a holistic approach to patient care in the treatment of acute conditions and management of chronic ones. Graduating from Springfield College in 2022 and a member of Pi Alpha National Honor Society, Avery mastered in the Science of Physician Assistant Studies and double minored in nutritional sciences and health care management.
Born and raised locally in Western Massachusetts, Avery enjoys being an active member and volunteer in our communities. With a zest for life, she enjoys staying active with her family and friends by taking walks, cooking, traveling, and playing pickleball.
Lauren Carra, PA-C
Lauren Carra is a board-certified physician assistant, who graduated from Springfield College in 2013. She is passionate about preventative primary care, and patient centered medicine. She has a passion for building relationships with her patients, and helping them live long, healthy lives. Lauren spent 7.5 years practicing hospital medicine at Mercy Medical Center and 2 years practicing medical oncology before she found her home at Personal Primary Care.
Lauren was born and raised in a small town in Maine and tries to stay authentic to her small-town roots. She resides with her husband and her two boys, Ben and Nolan. Her passion is running and training for road races, and motivating others to stay active and healthy. She enjoys to read, spend time with friends and family and cooking.
Erin Svrcek, PA-C
Erin Svrcek, PA-C, is a board-certified physician assistant and received her master’s degree from The George Washington University PA program in 2009. Born and raised in Western Massachusetts, she obtained her undergraduate degree in Biology from the University of Massachusetts. She is passionate about primary care and preventive medicine which has been the main focus of her practice. She also has experience in pulmonology, hospital medicine, urgent care and aesthetic medicine. She strives to provide compassionate care in a collaborative approach with patients to meet their healthcare goals.
Outside of the office, Erin is a wife and mom of two children. In her free time, she enjoys sports, yoga, reading, cooking and time spent outdoors.
Ashley Krupa, PA-C
Ashley Krupa, PA-C, is a board-certified physician assistant who is passionate about primary care and patient-centered medicine. She completed her Master of Science in Physician Assistant Studies at Springfield College. Ashley specializes in preventive medicine, chronic disease management, and obesity medicine. She emphasizes the importance of lifestyle modifications and regular screenings to promote long-term wellness. Her approach prioritizes empathy and collaboration, ensuring each patient receives a personalized treatment plan.
A lifelong resident of Western Massachusetts, Ashley takes pride in her Polish heritage. She graduated from the PA program alongside her twin sister. Outside of medicine, Ashley enjoys spending time with her three nephews, family, and friends. Her hobbies include reading, cycling on her Peloton bike, and visiting Cape Cod every summer.
Carolyn Burnham, PA-C
Carolyn Burnham is a board-certified Physician Assistant from Connecticut. Carolyn attended Springfield College where she received her Bachelor’s degree in 2022. In 2024, Carolyn completed her Master’s degree and graduated from Springfield College’s PA Program as a member of Pi Alpha National Honor Society. Throughout her training, Carolyn has developed a passion for creating meaningful patient connections. She strives to provide patient-centered care with a focus on preventative medicine.
While at home, she enjoys spending time with her family and friends, trying new foods, reading, and going on outdoor adventures with her two dogs Basil and Finn.
Samantha Birks, PA-C
Samantha Birks is a board-certified physician assistant who completed her studies at Springfield College where she earned a Master’s of Science in 2024 alongside a Bachelor’s degree in Health Sciences with a minor in Biology. She demonstrates passion for primary care medicine and recognizes the importance of approaching and preventing acute and chronic illness with holistic techniques and lifestyle modifications. She works to build strong patient-provider relationships that revolve around shared decision-making and are built on mutual respect and trust, with one goal in mind to improve health and quality of life.
Born and raised locally in Western Massachusetts, Samantha takes delight in spending time with family and friends, traveling to new and familiar places, reading, and incorporating movement into her daily routine to support healthy habits and embrace longevity in life.
Olivia Cyr, PA-C
Olivia Cyr PA-C is a board-certified physician assistant and a Springfield College graduate with a Bachelor’s Degree in Health Sciences and a minor in Biology. After she completed her Bachelor’s Degree she obtained her Master’s Degree and completed her requirements for the Physician Assistant program. Olivia is committed to patient focused care and assisting patients with meeting their health care goals and needs. She has a true devotion to developing trusting patient relationships.
Olivia grew up in Western Massachusetts and is excited to stay local within her career. Outside of her profession she enjoys spending quality time with her family, going to the gym and traveling to new places.
Hassen Borhot, NP
Hassen is a board-certified practitioner who has been in practice since 2010. He is an experienced clinician and has practiced in a wide variety of clinical settings including internal medicine, obesity medicine, addiction medicine, emergency, medicine, urgent care, and hospital medicine. He is passionate about preventative, evidenced-based medicine as well as healthy alternative lifestyles.
He obtained his undergraduate training locally at Elms College in Chicopee MA and his graduate education at the University of Connecticut in Storrs CT. He is licensed in both MA and CT. He is a local product of Western Massachusetts and still resides here.
