A general visit for established patients with your healthcare provider to address chronic health conditions.
Pre-Operative Visit
A visit with your healthcare provider for surgical clearance. Please come with pre-operative documentation from the surgeon to ensure appropriate and thorough care.
Sick Visit
A same- or next-day visit with your healthcare provider to evaluate and treat symptoms of a current ailment.
Annual Physical
A physical examination with your primary care provider focusing on wellness and prevention (Commercial Insurance only).
Weight Management Follow Up
A general weight loss visit for established patients for our weight management program to address medications, and lifestyle modifications.
Medicare Wellness Visit
A physical examination with your primary care provider focusing on wellness and prevention (Medicare insurance only, please fill out Medicare Wellness Packet prior to visit).
